Conceived as a training tool to teach his partner how to play Texas Hold'em Poker, Stephen Au-Yeung developed Casino Hold'em into a house game, and it first appeared as a live casino game in Egypt, Russia and South Africa in the late 1990s. The game became so popular that it was later showcased at the International Casino Exhibition in London in 2002 and was eventually licensed for use in the United Kingdom in 2007. The game is played with a standard 52-card deck, and each player makes an Ante bet and may also elect to make an optional AA Bonus side bet. After players and the dealer are both dealt two cards face down, three cards are then dealt to the board and will eventually contain five cards. Players have to decide whether to fold with no further play, losing their Ante bet, or to make a Call bet of double the Ante bet after checking their cards. If one or more players make a Call bet, the dealer will add two more cards to the board, for a total of five. Players and the dealer must form their best five-card poker hand from their two personal cards and five board cards.

The dealer must have a pair of 4s or better to qualify, and the dealer and the player's hands are compared. If the dealer doesn't qualify, the Ante bet pays according to the AnteWin pay table, and the Call bet is a stand off. If the dealer qualifies and the player's hand is better, the Ante bet pays in accordance with the Ante-Win pay table, and the Call bet pays 1 to 1. If the dealer qualifies and the dealer's hand is equal to the player's, all bets are stand offs. If the dealer qualifies and the dealer's hand is better, the player loses all bets.

No universal rule exists for optimal strategy for this game since the right decision for each hand is based on all seven cards that are dealt. While computer programs can quickly calculate the calculations needed to make the correct play in any given hand, it is still difficult to memorize a set of general rules as exists for Blackjack. However, the optimal strategy would be to raise 82% of the time, meaning that only in the worst 18% of hands should the player fold. These hands typically have two low singletons in the hole compared to the flop with little or no chance for a straight or flush.

In Casino Hold'em, players not only win a standard sum of 1 to 1 if they have the stronger hand, but they may also win even more for rare hands. The AnteWin pay table typically rewards a royal flush with a 100 to 1 payout, a straight flush with 20 to 1, four of a kind with 10 to 1, a full house with 3 to 1, a flush with 2 to 1, and a straight or less with a standard 1 to 1 payout.
